What your CGM measures beautifully
A CGM tracks glucose continuously, giving you a real-time picture of your blood sugar response throughout the day. It can show you:

- Whether your fasting glucose is trending up over time
- Patterns you'd never catch with a once-a-year lab draw
This is genuinely remarkable. A year of CGM data is more metabolic insight than most people get in a decade of annual physicals.
The question your CGM data is pointing toward
Here's where it gets interesting — and where your doctor conversation gets more valuable.
When your CGM shows stable blood sugar after a higher-carb meal, that's good information. But it naturally leads to a deeper question: what is your insulin doing to keep it stable?
Think of it like a sump pump in your basement. The floor is dry — that's your CGM reading. Everything looks fine. But the pump might be running constantly to keep it that way. The dry floor is good news. Knowing how hard the pump is working gives you the full picture.
Blood sugar and insulin work as a team. When cells start becoming less responsive to insulin's signal over time, the pancreas compensates by producing more insulin to get the same job done. The result: blood sugar stays stable, CGM looks great — but insulin is working harder than it should be.
This stage is called compensatory hyperinsulinemia, and it can exist for years before blood sugar levels show any change. Research has found that elevated fasting insulin strongly predicts the development of metabolic dysfunction years before glucose or A1C become abnormal — meaning fasting insulin is often the earlier signal.

The test most people have never had
Fasting insulin is not included in standard metabolic panels. It has to be specifically ordered — and most patients don't know to ask for it.
The standard lab reference range for fasting insulin is wide, partly because it's derived from a population that includes people with early-stage metabolic changes. Functional medicine practitioners typically look for a narrower optimal range, and research increasingly supports that approach for early detection.
In one study of young adults with completely normal blood glucose and normal A1C, nearly 30% had elevated insulin levels. Normal glucose. Normal A1C. But the insulin signal was already elevated — a pattern the CGM data alone wouldn't reveal.

Ask whether your fasting insulin is in the functional optimal range, not just the standard lab range
If you're seeing spikes after certain meals
Bring the pattern to your appointment — specific foods, times, and glucose curves
Ask whether the pattern warrants checking fasting insulin alongside A1C
Ask about the connection between post-meal spikes and longer-term metabolic trends
If you're just starting with a CGM
Log your symptoms alongside your glucose data — fatigue, brain fog, and cravings after meals are all meaningful context
Note your sleep and stress on high or low glucose days — the patterns are often connected
Bring 2–4 weeks of data to your doctor rather than isolated readings
The bigger picture
A CGM gives you a window into your metabolic health that simply didn't exist for most people a few years ago. Stable glucose lines are genuinely good news — they tell you your body is managing blood sugar effectively right now.
