I didn't know DEXA scans existed outside of bone density testing until I started digging into body composition. Now I think it's one of the most underutilized tools available to women โ and almost nobody talks about it.
What a DEXA scan actually measures
DEXA stands for Dual-Energy X-ray Absorptiometry. It takes about 10 minutes and uses less radiation than a cross-country flight. What it gives you that nothing else does:
Body composition โ not just your weight, but the precise ratio of lean muscle to fat throughout your entire body. It can tell you how much muscle you have in each leg separately, where fat is distributed, and whether your ratio is changing over time.
Visceral fat โ the fat stored around your organs. This is the fat most strongly associated with metabolic disease and inflammation. You can't see it, feel it, or estimate it accurately from the scale. DEXA measures it directly.
Bone density โ how strong your bones actually are. Bone density peaks in your late 20s and declines slowly after that, faster after menopause. DEXA catches the decline early.
Why BMI doesn't cut it
BMI is just your weight divided by your height. Two people can have identical BMIs โ one with 35% body fat and minimal muscle, one with 20% body fat and significant lean mass. The scale reads the same. The metabolic reality is completely different.
There's actually a term for this: "normal weight obesity." It describes people whose BMI looks fine but whose body composition tells a different story. More common than most people realize โ particularly in women over 40 who've lost muscle mass without gaining visible weight.

- Family history of osteoporosis
- Trying to build muscle and want objective data on whether it's working
- Weight has been stable but you suspect body composition has shifted
- Metabolic concerns and you want to understand visceral fat specifically

The trend over time is often more useful than a single number.

Radiology centers and hospitals offer them โ often covered by insurance for bone density if you meet criteria. Functional medicine and longevity clinics offer body composition DEXA specifically. Out of pocket, a full body composition scan typically runs $50โ150.
