Your cholesterol looks fine. Your blood pressure is normal. But there's a marker that's strongly associated with cardiovascular risk, cognitive decline, and inflammation — and most standard panels don't include it.
What it is
Homocysteine is an amino acid that builds up in your blood when certain B vitamins are low or when a genetic variant called MTHFR impairs their processing. In a body that's methylating well, it gets converted quickly. When that conversion is impaired — from low B12, low folate, low B6, or an MTHFR variant — it accumulates.
Elevated homocysteine is associated with increased cardiovascular risk, stroke risk, cognitive decline, and inflammation — independently of cholesterol. You can have a perfect cholesterol panel and elevated homocysteine at the same time.
Why it's worth knowing about
What I find most interesting about homocysteine is that it's a downstream signal. When it's elevated, it's telling you something upstream isn't working — usually a B vitamin deficiency or impaired methylation. That makes it one of the most actionable markers out there. Address the upstream cause and homocysteine typically comes down.

What the numbers mean

Conventional labs often only flag it above 15. Functional medicine practitioners tend to aim for below 7–9 based on research associating lower levels with reduced cardiovascular and cognitive risk.

Worth bringing up if you have a family history of cardiovascular disease or dementia, unexplained fatigue or brain fog, known B12 or folate issues, an MTHFR variant, or elevated inflammation markers without a clear cause.
