I fell down a rabbit hole on this one โ and I'm glad I did, because it explained something that had confused me for a long time.
Have you ever taken B12 faithfully, retested, seen your levels go up, and still felt exactly the same? There's a reason for that. And it has four letters: MTHFR.
What it is โ in plain English
MTHFR is a gene that produces an enzyme your body uses to convert folate into its active, usable form. A variant in that gene can reduce how efficiently that conversion happens.
Why does it matter? Because the active form of folate โ methylfolate โ is essential for something called methylation. Methylation affects nearly everything: DNA repair, mood regulation, detoxification, cardiovascular health, and how your body processes B vitamins. When methylation isn't working well, the downstream effects show up in a lot of different ways.
The two most common variants
C677T
The most studied variant. Having one copy reduces enzyme activity by roughly 40%. Having two copies reduces it by up to 70%.
A1298C
A second variant that can compound things when found alongside C677T. Roughly 40โ60% of the population carries at least one variant. Most never know.
Why your supplement might not be helping
Here's the thing I didn't know until I dug into this: most B12 supplements contain cyanocobalamin โ a synthetic form your body has to convert before it can use. Most folic acid supplements are the same โ synthetic, requiring conversion via the very enzyme that isn't working efficiently.
So the supplement takes the number up on a lab test. The cellular deficiency stays. You feel the same. And nobody connects the dots.
The folic acid problem hiding in plain sight
This is the part that really got me. In 1998 the US mandated that enriched grain products โ bread, pasta, rice, cereals โ be fortified with folic acid. The intention was good: reduce neural tube defects in pregnancy.
The unintended consequence: Americans are now consuming significant amounts of synthetic folic acid through everyday food without realizing it.
For people with MTHFR variants, synthetic folic acid actually competes with natural folate for the same receptors โ and because it can't be efficiently converted, it can block absorption of the folate your body could actually use. You're eating fortified bread, fortified pasta, fortified cereal โ and the fortification itself may be working against you.
European bread and pasta are largely not fortified this way. The folic acid mandate is an American policy โ which is one reason this comes up so often here.
