What "normal" actually means
Conventional lab ranges are built from population averages. They're designed to catch disease — not to identify the gap between feeling sick and feeling genuinely well.
The way I think about it: "normal" is the speed limit. It tells you the legal boundary. It doesn't tell you the optimal speed for your particular car, your tires, your weather, and your destination.
Your labs being in range means you're not in a medical crisis. It doesn't mean you're operating at your best.
Where the gap shows up
Ferritin
Conventional normal: 12–150 ng/mL · Functional optimal for women: 50–100 ng/mL
A ferritin of 14 is "normal." Most women feel exhausted and lose hair below 50.
TSH (Thyroid)
Conventional normal: 0.5–4.5 mIU/L · Functional optimal: 1.0–2.0 mIU/L
A TSH of 4.2 is "normal." Many people feel cold, fatigued, and foggy at that level.
Fasting Glucose
Conventional normal: below 100 mg/dL · Functional optimal: 70–85 mg/dL
A fasting glucose of 97 is "normal." At that level, insulin dysregulation may already be developing.
Vitamin D
Conventional normal: above 20 ng/mL · Functional optimal: 50–80 ng/mL
A Vitamin D of 22 is "normal." Most people don't feel the benefits until they're above 50.
B12
Conventional normal: above 200 pg/mL · Functional optimal: above 500 pg/mL
A B12 of 250 is "normal." Neurological symptoms and fatigue are common below 500.
This isn't about doctors being wrong
I want to be clear about this — because it's easy to read this kind of information and feel frustrated with your doctor. Conventional ranges exist for a reason. They're validated, standardized, and appropriate for catching disease.
Functional ranges are a different lens — one focused on how you feel, not just whether you're sick. Neither is wrong. They're answering different questions. The question worth asking isn't just "is my lab normal?" It's "where in the range am I — and could that explain how I feel?"
